Osteoclast-Like Giant Cell Carcinoma Hidden by a Bladder Stone in a Patient With Neurogenic Bladder

초록 | Neurogenic bladder patients with emptying disorders not performing clean intermittent self catheterization (CIC) are at increased risk of urinary tract infections and bladder stones. While on CIC they are at increased risk of chronic inflammatory diseases and bladder neoplasm due to the procedure itself. We report the case of a 54-year-old woman with late diagnosis of neurogenic bladder due to infant meningitis. She came at our observation for filling phase lower urinary tract symptoms (LUTS). She was diagnosed of a bladder stone following CT scan and underwent open cistolithotomy. Inside the bladder some amorphous material was found that at final pathology was an undifferentiated fusocellular neoplasia with osteoclast like giant cells. She underwent radical cistectomy and the final histology confirmed the initial diagnosis. Osteoclastic giant-cell tumor of the bladder is an anecdotic entity. Its ability to resemble an osseous structure, mainly at radiologic imaging, could induce an erroneous diagnosis. In patients with possible risk factors of bladder neoplasm it may be advisable to consider performing a cistocopy before open surgery to avoid the risk of a delay in the treatment of the primary disease. |
